Medical Chart Auditor

 We are seeking an experienced and detail-oriented Medical Chart Auditor to review and analyze clinical documentation for accuracy, completeness, and compliance. The ideal candidate will have strong knowledge of medical coding, billing, and healthcare regulations, ensuring that all patient charts meet internal standards and external compliance requirements. This role plays a critical part in maintaining quality assurance and supporting accurate reimbursement processes. 

• Review and audit patient medical charts for accuracy, completeness, and compliance with CMS and payer guidelines.
• Verify the accuracy of coding, documentation, and billing practices.
• Identify discrepancies and provide recommendations for correction and improvement.
• Collaborate with clinical and administrative staff to resolve documentation or coding issues.
• Prepare detailed audit reports summarizing findings, trends, and corrective actions.
• Educate staff on documentation standards and compliance requirements.
• Maintain updated knowledge of healthcare regulations, coding changes, and payer policies.
• Participate in quality assurance initiatives to improve charting and billing accuracy.
• Protect patient confidentiality and adhere to HIPAA compliance at all times.
• Support internal and external audits as needed.

• Certified Professional Coder (CPC), Certified Medical Auditor (CMA), or equivalent credential preferred.
• Minimum of 2 years’ experience in medical chart auditing, coding, or healthcare compliance.
• Strong understanding of ICD-10, CPT, HCPCS, and CMS guidelines.
• Excellent analytical and attention-to-detail skills.
• Strong communication and report-writing abilities.
• Proficient in EHR systems and Microsoft Office applications.
• Ability to work independently and meet tight deadlines.
• High level of integrity and confidentiality in handling sensitive information.

• Computer or Laptop – Minimum 8GB RAM and reliable processor (i5 or above).
• Stable Internet Connection – At least 20 Mbps download speed with backup access.
• Noise-Cancelling Headset – For communication with internal teams and clients.
• External Webcam – For virtual meetings and audit discussions.
• Backup Power Source – Such as a UPS or power bank to avoid interruptions.
